Each entity receives a composite score on a 0–100 scale:
- KPI Achievement (70%) — actual vs target, weighted by KPI importance
- Timeliness (15%) — whether submissions met the deadline
- Evidence Quality (10%) — strength of supporting documentation
- Improvement Trend (5%) — period-over-period progress
Ratings: Excellent ≥ 90 Good ≥ 75 Satisfactory ≥ 60 Poor ≥ 40 Unsatisfactory < 40
